Social Care Cloud. Reduce bed-blocking, accelerating social care.

After fifteen years of healthtech innovation at Being Guided I am now introducing a new innovation: Vibe Coding on Salesforce and Microsoft technologies.

At Being Guided we understand that any investment in digital innovations within the NHS must deliver meaningful, measurable value. This includes future integrations with the forthcoming Single Patient Record (SPR) Web apps being defined for UK NHS.

So the first step we take with our customers is Mutual Discovery applying Design Thinking1 and Value Engineering2 to result in true value of investing in AI-powered digital innovation.

During a Mutual Value Discovery we combine Design Thinking and Value Engineering with Vibe Coding which creates a solid foundation to engage all stakeholders, create a robust financial justification and design a Web app implementation that works for everyone.

Proven Technology.

At Being Guided, this work builds on the early adopter success we have had with UK NHS Trusts, including Moorfields Eye Hospital NHS Foundation Trust and in earlier engagements with Sistema Nacional de Salud (SNS) in Spain.

Social Care Cloud is a current example. This is focused on reducing bed-blocking in NHS hospitals by placing the citizen as patient is at the centre of communications and accelerating the hospital dicharge-to-care process. This includes the option to integrate with Electronic Health Records (EHR) and Social Care Case Management Records.

Many readers from the UK NHS will likely be more familiar with Micosoft technology: Office 365; Azure; and, more recently, AI and Copilot. As illustrated below, and as described in my Journal post Vibe, the combination of Power Apps, Dataverse and Azure becomes a solid foundation with AI Builder (Copilot).

These Social Care Plans can be integrated with Electronic Health Records (EHR) but this is not required for initial implementations. For integration we have extensive experience with combining AI=powered No-Code solutions with EHR systems at the UK NHS or internationally.

The technology allows independent creatives (artists, musicians, etc.) to submit Schedules and generate Invoices for payment for the delivery of Social Prescribing. This includes Schedules in-hospital and post-hospital, at home or in a community environment.

Social Care delivered by agencies, volunteers or family members may be added to an integrated Social Care Plan that will include Social Prescribing.

For creatives, the technology allows for artists and musicians to submit Schedules, Invoices and Reports via simple Forms on desktop, laptop, tablet or smartphone devices.

The Social Care Cloud comprises:

People.
With citizens viewed as Customers, Social Care Cloud is delivered on an AI-powered No-Code Platform. This is putting People first and enabling patients, families and the widest community of nonprofit, creative and community organisations to be at the centre of what matters most: the health and wellbeing of citizens and professionals.

Process.
Each Citizen record is updated by patients or nominated family, friends or carers via simple set of Web forms, avoiding the need for software licences and making this highly afforadale at scale. All data is shared through citizen consent, and carefully controlling which stakeholders may edit or view different record field-level content.

Plan.
A Social Care Plan integrates a post-hospital Care Package with Social Prescribing, including a Schedule of Performer activities that run in hospital, community or other safe locations. Patients, family, friends, volunteers, careers and creatives (artists, musicians, etc.) can provide appropriate updates to Social Care Plans, via Web Forms on smartphone, tablet, laptop or desktop devices.

Performer.
A network of Performers can be added to Social Care Cloud, allowing Schedules for Social Prescribing activities to be submitted and approved. When completed, this provides each artist, musician or other Performer to generate and receive Invoices for payment by the appropriate stakeholder as budget holder.

Platform.
Platform is built on a choice of AI-powered technologies: Salesforce; Google; or, Microsoft. This is for a combination of Professional Users and Citizen Users (patients, families, creatives, carers and volunteers) plus Performers (eg. artists, musicians, etc.). As illustrated above, this enables a broad array of stakeholders, with appropriate permissions, to update Social Care Cloud records.

Advantages to the Social Care Agency.

Whilst the NHS gain significant advantage through eliminating bed-blocking and accelerating otherwise delayed patient operations, it is important to look at the win-win benefits that also accrue to the social care agency with the Social Care Cloud.

Winning Local Authority Tenders.
In the current commissioning landscape, 'Digital Maturity' is no longer optional; it is a scored criteria in tender bids. Local Authorities and NHS Integrated Care Boards (ICBs) are actively moving away from paper-based providers. By investing in the Social Care Cloud, social care agencies offer commitmrnt to solid, safe 'Digital Social Care Record' (DSCR) compliance and data security (DSPT) standards

Reduction in Staff Churn and Recruitment Costs.
Staff turnover in social care is approximately 29%3, and it costs roughly £3,500 to recruit and train a single new care worker. Care workers leave because they hate unpaid admininstration work and feeling unsupported. The Social Care Cloud cuts their paperwork by digitizing handovers and care notes, allowing them to do the job they enjoy.

Mastering Discharge to Assess (D2A) Revenue.
The NHS Discharge to Assess (D2A) Model funds up to 6 weeks of care to get patients out of hospital faster. The agency that responds fastest gets the patient (and the funding).With the Social Care Cloud, agencies can accept the Referral, view the Care Plan, and roster the visit in minutes. You will capture the volume that slower, analogue agencies miss."

Justifying Higher Rates for Private Payers.
For private families, peace of mind is the product. My Social Care Cloud gives family members a 'window' into their loved one’s care—real-time updates on visits, medications, and mood.

Unlocking Social Prescribing as a Differentiator.

Social Prescribing (referring patients to non-clinical services from creative Performers) is a way to deliver care that is highly valued by patients and insurers for preventing decline.

Adding the Power of AI.

Intelligent Discharge Summaries: Using Microsoft Azure OpenAI Service integrated with Power Apps, the system can parse complex Electronic Health Records (EHR) to draft Discharge-to-Care Summaries. This augments the health professional's ability to communicate post-hospital needs to social care agencies without manual drafting (GOV.UK, 20254).

Sentiment and Mood Tracking: Copilot can analyse unstructured feedback from Web forms submitted by family members or carers, flagging subtle shifts in a patient’s wellbeing or mood, that might otherwise be missed by a time-pressed social worker (Care Computers, 20255).

Social Prescribing Matching: For Performers (artists/musicians), AI prediction models can suggest the most appropriate creative activities based on a citizen’s mood, history, or their documented clinical recovery goals.

Automated Document Processing (D2A Revenue): AI-powered Form Processing automatically extracts data from Performer invoices or paper-based care notes. This accelerates the "Discharge to Assess" (D2A) revenue cycle by turning static schedules into digital records in Dataverse without manual entry (MegaMinds Technologies, 20256).

Predictive Bed-Blocking Mitigation: By analyzing historical flow data in Azure, AI can predict which patients are at high risk of Delayed Transfer of Care (DToC). It can then trigger Power Automate workflows to pre-alert social care agencies or roster community volunteers before the discharge is even finalised.

Self-Service Onboarding: For independent Careers or Performers, AI-driven chatbots can handle the submission of Schedules and Invoices, answering queries about payment status or reporting requirements, thus reducing the burden on Local Authority staff (Care Computers, 20255).
